Transition Specialist

Key Responsibilities

Proactively make outbound calls and manage intake processes for new patients.

Communicate proper expectations to patients

Communicate functionality and expected utilization of monitoring equipment

Ensure accurate record management by obtaining and communicating relevant patient information, patient history, insurance details, and intake forms.

Qualifications and Skills:

Inside or outside healthcare sales, preferred

Proficient in data entry and Microsoft tools

Demonstrates a high level of customer service and interpersonal communication skills

Contact center or other high call volume experience, preferred

Highly detail-oriented

Ability to multi-task in a fast-paced work environment

Healthcare experience, preferred
