About Us

Healthcare is not just a business to us. Improving the quality of life for our patients is our PURPOSE!

On day one we made a decision.
“If we put patients first, everything else will fall into place”

To integrate into the patients’ existing healthcare team, offering additional value to both patient and healthcare providers.

Our fundamental goal is to provide a level of monitoring and clinical intervention that is unique to healthcare and fills in the healthcare gaps. We deliver individualized holistic patient care while connecting in a way that’s effective for the patient. We treat all patients with dignity and respect and pride ourselves on treating patients that are underserved, deemed non-compliant and are high risk with multiple co-morbidities. We meet all patients where they are in their healthcare journey.

Core Values

Patient centered healthcare: Passion in serving the needs of the community is our top priority and we believe in doing that to the best of our ability. We put patients first, meet them where they are, and create accessibility to healthcare with innovative IT solutions in order to manage and improve quality of life.

Strong Partnerships: Through integrity, teamwork, and trust, we cultivate strong parternships. We advocate for our patients together to provide them with the best care and overall success.

Company Culture: We believe there is no better working environment than a positive one. Employees are nurtured for success by strong leadership, challenging their skill set, allowing room for growth, and feeling valued and appreciated. The best companies set goals, work hard, and find ways to have fun.
